Your First Appointment

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records

Please refer to our insurance list for physician participation. It is important that you check with your insurance plan to determine if your office visit will be considered in-network or out-of-network.

Please bring the following information with you to your appointment:

  • Insurance card
  • Referral (if necessary)
  • Prior testing, including: X-rays/MRIs/CAT scan films, and the medical reports that belong with each

Please arrive a few minutes prior to your appointment time to complete necessary paperwork.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible it will be necessary to pay at the time of your visit.

Payment will also be required for all non-covered services. For your convenience we accept cash, checks, Visa and Mastercard.

If your injury is work-related, you must provide us with the name, address and telephone number of the worker's compensation insurance carrier. We also require the date of the accident and worker's compensation claim number. If your claim is denied by the insurance carrier, you are responsible for payment of your bill.

If your injury is due to a motor vehicle accident, you must provide us with the name, address, and telephone number of the no-fault insurance carrier, and the name of the insured person. In addition we will require the no-fault carrier claim number.

If your claim is denied by the insurance carrier, you are responsible for payment of your bill.

If your injury is school-related, you must provide us with the name, address and telephone number of the school's insurance carrier and a claim number for this injury. If your claim is denied by the insurance carrier, you are responsible for payment of your bill.

Referrals

If your insurance is an HMO/Managed Care Plan and you are required to obtain a referral from your Primary Care Physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your Primary Care Physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

Cancellations

If you are unable to keep your appointment or are going to be delayed, please call our office.

Prescriptions and Refills

Please call our office during office hours for prescriptions and refills. In order to comply with your request, we will need to refer to your medical records. Depending on when you were last seen in the office, your physician may recommend an office visit for re-evaluation prior to dispensing new medications.

Emergencies

Our physicians are affiliated with North Shore University Hospital at Manhasset and St. Francis Hospital in Roslyn. In case of an emergency, and the office is closed, please go to the emergency department at either of these hospitals and request that a physician from our practice be called. Some emergencies can be handled in our office. If our office is open, please call our office before going to the emergency room.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients and a copy is available upon request.

Massage Therapy

We are happy to announce that we are now offering Massage Therapy. Please inquire for all details at the Physical Therapy front desk.
