Please call ahead to schedule. Weekend walk-in care is now offered in Great Neck on Saturdays and Sundays from 12:00 to 4:00 pm.

To submit a request, please visit HealthMark’s Request Manager by clicking the button below. Once you have logged in, select “Submit Request” from the menu and fill in all required fields to authorize HealthMark directly. Your request will then be processed, and you will receive an email notification once it is complete and available for download.
