Orthopaedic Associates of Manhasset has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests. Please follow the instructions below to submit your request. For legal medical records requests, please call our office at (516) 627-8717.

Submit a Request

To submit a request, please visit HealthMark’s Request Manager by clicking the button below. Once you have logged in, select “Submit Request” from the menu and fill in all required fields to authorize HealthMark directly. Your request will then be processed, and you will receive an email notification once it is complete and available for download.
